Medical Records Requests
Requests for medical records are processed by HealthPort – a full service health document processor located on site at Ogden Clinic.
To obtain records you must complete a release form by:
- Click Here for printable form.
- Stop by any Ogden Clinic location and ask any receptionist for the form
- For continuing care with a medical provider outside of Ogden Clinic, complete a form at your doctor's office and ask them to fax it to Ogden Clinic at 801-475-3454
