Pre-Registration

Thank you for pre-registering. Once we have your information, we will be prepared for your visit.

Please review the following important information:

1. Please arrive 5 - 10 minutes early for your appointment.
2. Please bring your current insurance card with you to each appointment.
3. Remember to bring your co-payment, deductible, or co-insurance amounts as required by your insurance plan.

If you do not have insurance, please be prepared to pay for your visit at the time services are provided.

You can pre-register three ways:
• Register online using the secure form. Online registration must be done at least 12 business hours in advance.
• Print the registration form, fill it out at home and bring it in at the time of the appointment.
