How to access Year-end Tax Statements

The button below directs active and former employees to retrieve their year-end tax forms (W-2 and 1095-C). Please refer to the instructions to navigate and download your documents.

Retrieve Tax Statements

Active Employees

Log into UKG using your existing credentials. Tax forms are found at:

• W‑2 forms: Go to Pay → U.S. Wage and Tax Statements
• 1095‑C forms: Go to Benefits → 1095‑C

Former Employees

Former employees will still receive tax statements from Ogden Clinic for the period they were employed. Please follow these instructions to retrieve them:

Login credentials:

• Username: Ogden Clinic email address firstname.lastname@ogdenclinic.com
• Password: your birth date in MMDDYYYY format (no spaces or /’s)

After logging in:

• W-2 forms: Go to Pay → U.S. Wage and Tax Statements
• 1095-C forms: Go to Benefits → 1095-C
