Telehealth Jurisdictions

Ogden Clinic provides telehealth services only to patients located in the State of Utah at the time of their telehealth visit. Our providers are licensed to practice medicine in Utah, and telehealth services are not available to patients physically located outside of this jurisdiction. Patients are responsible for confirming their location at the start of each telehealth encounter.

Provider Identity

Patients will be informed of the identity and credentials of the medical provider responsible for their care at or before the time services are delivered. Information disclosed includes the provider’s name, professional title, and specialty. This information is shared verbally during in-person and telehealth encounters and is also displayed on patient materials such as appointment confirmations, clinical notes, or the Ogden Clinic provider directory.

II. Notice of Privacy Practices (HIPAA)

This section explains how Ogden Clinic uses and discloses your Protected Health Information (PHI) and your rights under HIPAA.

Who We Are

This Notice applies to Ogden Clinic, its providers, employees, staff, and facilities.

Our Privacy Obligations

We are required by law to:

  • Maintain the privacy and security of your PHI.
  • Provide this Notice of our legal duties and privacy practices.
  • Notify you following a breach of unsecured PHI.
  • Abide by the terms of this Notice currently in effect.

How We May Use and Disclose PHI

With Your Consent

  • Treatment: Providing and coordinating your care, including appointment reminders and treatment alternatives.
  • Payment: Billing and collecting payment from you, your insurer, or other payers.
  • Healthcare Operations: Internal operations such as quality improvement, staff training, and customer service.
    You may restrict certain disclosures to health plans if you pay in full out of pocket for a service.

With Your Authorization
We will not use or disclose PHI for purposes beyond those above without your written authorization. You may revoke an authorization at any time in writing.

Without Your Consent or Authorization
We may disclose PHI without prior consent when permitted or required by law, including:

  • Emergencies.
  • Public health activities (disease reporting, child abuse/neglect, FDA requirements, workplace safety).
  • Victims of abuse, neglect, or domestic violence.
  • Health oversight activities (audits, Medicare/Medicaid).
  • Judicial or administrative proceedings.
  • Law enforcement purposes.
  • To prevent or reduce a serious threat to health or safety.
  • Specialized government functions (military, national security).
  • Coroners, medical examiners, and funeral directors.
  • Research, with proper safeguards and/or IRB approval.
    We may also disclose PHI to family members or close friends involved in your care if you agree, do not object, or we reasonably infer that you would not object. If you are incapacitated, we may disclose information if determined to be in your best interest.

III. Your Rights Regarding PHI

You have the right to:

  • Request Restrictions on certain uses and disclosures (though we may not be required to agree).
  • Confidential Communications – request communication in a specific manner or location.
  • Inspect and Copy your health and billing records (fees may apply).
  • Amend Records you believe are incomplete or inaccurate.
  • Accounting of Disclosures – request a list of certain disclosures made in the past six years.
  • Receive a Paper Copy of this Notice, even if you received it electronically.
  • File a Complaint if you believe your rights have been violated. You will not face retaliation.

IV. Changes to This Notice

We reserve the right to change this Notice at any time. Updates will apply to all PHI we maintain. Revised Notices will be posted in our facilities and on www.ogdenclinic.com.
