Your first visit to Utah Hematology Oncology | Ogden Clinic may come at
a time filled with anxiety as you face a tremendous challenge. We would
like to make your first and every visit as pleasant as possible.
Since this can be a very emotional time, we strongly recommend taking a
few actions that will help you get the most out of your visit:
Consider having a family member or friend accompany you who can take careful
notes. This is also true for when you receive education from providers
and nurses. There is a lot to learn and so much happens in such a short
time it is easy to become overwhelmed or confused.
Please bring your government-issued photo ID; your current insurance card(s)—including
pharmacy card, if different; the completed forms; and your initial copay
(see your card or contact your insurance company for details).

here prior to your first visit. This helps us obtain your health information
and streamline the patient intake process.
